CA2 – Patients fill this out when they need to report an injury or condition to their supervisor that happened over 2 or more shifts, 2 or more days, weeks, months, or years. This is filled out at and with the agency / supervisor. We want to see a copy of this during intake so we can understand what has happened to the patient. Key pieces of information are also found here: DOB, DOI, Last Name (all needed for EComp and for looking up patient cases on ACS). Information from the CA2 is used to write medical narratives to get injury claims accepted.
Common CA2 conditions include carpal tunnel syndrome, tendonitis, osteo arthritis, and other physical, emotional, psychological conditions that develop over 2 or more work shifts, 2 or more days, 2 or more weeks, 2 or more years, 2 or more decades.
The CA2 consists of 4 pages. Page 1 is for the patient to fill out. Page 2 is for the supervisor. Page 3 is the Notice of Receipt that patients need in order to prove they reported their injuries on time. Page 4 is an instruction sheet.
This form is filled out by the injured employee and the supervisor (sometimes HR). The CA2 has no place for the doctor to fill out or sign. For doctors, this form is for informational purposes only.
