OWCP 915 – This Claim for Medical Reimbursement is used for patients who have previous out-of-pocket medical expenses related to their OWCP work injury. These expenses were usually incurred prior to the claim being approved by OWCP, but some expenses may also be incurred after. Always make these forms available to patients in the office. Patients must fill this out themselves and attach copies of all receipts from all medical expenses related to their work injury (copays, deductibles, etc.). These forms and their supporting documentation are sent into the claims examiner for reimbursement. As with all OWCP benefits, these forms cannot be submitted until the OWCP claim is accepted by the claims examiner.
