OWCP 957 – This Medical Travel Refund Request is used for patients who want to be reimbursed for travel to and from medical / therapy / diagnostic visits related to an OWCP work injury. Patients can be reimbursed for roundtrip miles (up to 100 miles round trip), taxi rides, Uber / Lyft rides, bus / train rides, lodging, meals, and other. Always recommend to patients that they talk to their claims examiners prior to incurring these expenses to try to get preauthorization. Make these forms available to patients in the office. Patients must fill this out themselves and attach travel receipts and proof of mileage (Google maps, etc.). These forms and their supporting documentation are sent into the claims examiner for reimbursement. As with all OWCP benefits, these forms cannot be submitted until the OWCP claim is accepted by the claims examiner.
