OWCP 1168 – Provider Enrollment Form. This is the form used for doctors and other healthcare professionals to be credentialed by the Department of Labor to help injured federal employees. If the form has any errors on it, credentialing will be denied and returned to the office. Always attach supporting documents such as the doctors license and the licenses of other healthcare providers involved in the practice. Follow the instructions attached to the OWCP 1168 form.
