BILLING DOCUMENTATION

Effective documentation for billing should include efficiency, accuracy, and compliance. The included forms meet all that criteria. Each doctor / office may offer a range of treatment options not listed in these forms. The forms are a sample that can and should be modified to fit each practice’s unique treatment options. Each doctor should determine his/her own treatment philosophy and documentation should reflect that.
